Charlie Sheen has stepped into a legal mess after a visit to the dentist reportedly turned violent.

According to a lawsuit filed by Margarita Palestino, a dental technician based in Los Angeles, the actor, 49, committed assault, battery, sexual battery and intentional infliction of emotional distress on Sept. 25, when he sought treatment at his dentist's office for an abscess.

In her complaint, filed Friday in the Superior Court of the State of California, Palestino states that she was assisting Drs. George Bogen and Jaime Azdair in an examination room when Sheen ripped off the nitrous oxide mask she had placed on him upon Bogen's orders, and then shouted, "I'm going to f–––ing kill you!" to all who were present in the room.

When Bogen asked her to replace the mask, Sheen responded by grabbing Palestino's "left breast area" and then "grabbed her bra strap, and forcefully pulled it down in an attempt to open or remove her bra," according to the papers obtained by PEOPLE. She fled from the examination room when she was able to break free of Sheen, she claims.

Palestino also claims in the papers that Sheen threatened Azdair with a knife, though she was not in the examination room at the time. After re-entering the examination room at the request of Bogen, Palestino claims the room was in disarray, with spit covering the walls.

Sheen refused to stay seated for a topical anesthetic to take affect, according to the legal documents, and "violently bolted out of the dental chair, and forcefully punched [Palestino's] chest."

According to the lawsuit, before Sheen left the office, his personal assistant and body guard – who was present at the time of the incidents – handed Palestino $500 and said, "This is for your troubles." She also claims she was made to clean the spit off the walls by Bogen, who told her that Sheen was "high on a combination of crack cocaine, Theradol and alcohol" during his visit.

Palestino is asking for compensation for "physical injury, pain and suffering, and mental anguish." She also demands a trial by jury.

A representative for Sheen could not immediately be reached for comment on the lawsuit. However, his publicist told PEOPLE previously that the actor "had a bad reaction" to the gas administered at the dentists' office and denied a knife or illegal drugs were involved.
