Camille Grammer was granted a temporary restraining order against her boyfriend Dimitri Charalambopoulos on Tuesday after she says he assaulted her in a Houston hotel room.

The Real Housewives of Beverly Hills star claims that Charalambopoulos, 36, repeatedly slammed her head into furniture during the altercation, which took place on Oct. 16, just five days after she underwent surgery for endometrial cancer.

In the court filings obtained by PEOPLE, Grammer, 45, stated that the argument began at 1:30 in the morning when Charalambopoulos, her boyfriend of two years, received text messages and phone calls "from another woman."

As the fight escalated, "He grabbed me by the hair, twisted it around his hand, and repeatedly pulled my head into the bed and its headboard," Grammer says. "[He] succeeded in pulling out swaths of my hair and gave me a large, painful contusion on the side of my head. All the while, I was screaming for help."

Grammer, who was recovering from a radical hysterectomy, asserts that the 6'2" fitness trainer squeezed her nose and covered her mouth in an attempt to silence her and prevent her from breathing. "I began to believe he would kill me," she said.

She states that the assault continued for between 40 and 60 minutes as he wrestled her onto the bed, pinning her down by her wrists and kneeling on her stomach "mere inches" from where she'd had surgery days earlier.

The Superior Court judge granted her a restraining order that states that Charalambopoulos cannot contact or come within 100 yards of her or her two children – Mason, 12, and Jude, 9 – with ex-husband, former Frasier star Kelsey Grammer.

The court filing also includes pictures of bruises on Grammer's wrists, hair pulled from her head, bruises on her face and her smashed iPhone.

Grammer went to the doctor hours after the attack as she feared she would suffer internal bleeding or complications from her recent surgery.

"I wake up at least twice per night with visions of his face as he was attacking me," she said. "I continue to fear for my safety and for the safety of my children."

Efforts to reach Charalambopoulos were unsuccessful.
